Member Username Request

Please enter your Member Number and Email Address, then click on the ‘Get Username’ button.

Please note that all member numbers are six digits long and may contain an alphabetic character. For example, the primary member number is 001234, the spouse member number is 001234A, the oldest dependent member is 001234B, etc.
